Humana Healthy Horizons is looking for RN, Field Care Managers to conduct both telephonic and in-person assessments of our long-term services and support (LTSS) and aged, blind, and disabled (ABD) member population. In this role, you will play a vital part in helping members and their families connect with essential resources, ensuring optimal wellness and facilitating supportive care.
Your responsibilities include visiting members in their homes or preferred locations, taking the time to understand their individual needs, goals, and challenges, and connecting them with quality services that enhance their well-being and promote positive health outcomes.
• Review and assess medical complexity for individual members.
• Create personalized care plans tailored to each member's needs.
• Utilize evidence-based strategies and techniques to manage physical, environmental, and psychosocial health.
• Identify hurdles affecting care delivery and proactively implement solutions.
• Continuously monitor member progress towards health outcomes through comprehensive assessments.
• Make informed, independent decisions in uncertain situations.
• Navigate organizational strategies effectively, understanding their relation to overall objectives.
• Adhere to established guidelines and procedures for quality care delivery.

Required Qualifications:
• Must reside in Virginia (NOVA area).
• Active Registered Nurse (RN) license in Virginia with no disciplinary actions.
• Two (2) years of healthcare and/or case management experience.
• One (1) year of experience working with individuals meeting Cardinal Care Priority Population criteria (adults and children at risk for chronic medical conditions and high social needs).
• Familiarity with community health resources and agencies.
• Intermediate to advanced computer skills, particularly with Microsoft Office.
• Proficiency in using electronic health records and various information applications.
• Exceptional communication and interpersonal skills, with the ability to build rapport quickly.
• Ability to work autonomously with minimal supervision.
• This role requires travel to meet with members and their families for assessments.
• Must have a valid driver's license and maintain vehicle insurance according to state requirements.
Preferred Qualifications:
• Bachelor of Science in Nursing (BSN).
• Certification in Case Management (CCM).
• Previous experience in managed care.
• Motivational Interviewing Certification and/or knowledge.
• 3-5 years of in-home assessment and care coordination experience.
• Experience in disease management, health coaching, and wellness initiatives.
• Bilingual or Multilingual proficiency (English/Spanish, Arabic, Vietnamese, Amharic, Urdu, etc.).
Additional Information:
• Workstyle: Field position providing services at non-company locations, including member homes.
• Travel: 50 - 75% of the time interacting with members, their families, and providers.
• Workdays and Hours: Monday - Friday; 8:00am - 5:00pm Eastern Standard Time (EST).
• Language Assessment: Candidates who communicate in a language other than English will be required to undergo a language proficiency assessment.
Scheduled Weekly Hours: 40
Pay Range: $71,100 - $97,800 per year. This position is also eligible for a bonus incentive plan based on performance.
