Registered Nurse (RN) – Full-Time Facility: Hanford Post Acute Location: Hanford, CA Salary Range: $40.00 – $45.00 per hour (BOE) Status: Full-Time Shifts: As Assigned General Purpose The primary responsibility of the Registered Nurse (RN) is to supervise and coordinate the day-to-day nursing activities of the facility during the assigned tour of duty. This position ensures compliance with all federal, state, and local standards, regulations, and guidelines governing skilled nursing facilities, and works under the direction of the Director of Nursing (DON) and/or Assistant Director of Nursing (ADON) to ensure the highest level of quality resident care. Essential Duties and Responsibilities Every effort has been made to identify the essential functions of this position; however, this is not intended to be an exhaustive list. Duties may be expanded as needed to meet facility and resident care needs. Administrative Functions • Assist the DON/ADON in directing daily nursing operations in compliance with regulatory requirements • Participate in developing, maintaining, and updating nursing policies and procedures • Ensure nursing reference materials and procedure manuals are current and accessible • Monitor compliance with nursing service policies and procedures • Prepare written and oral reports regarding nursing operations • Assist with discharge planning and coordination of nursing services • Participate in quality assurance and performance improvement (QAPI) initiatives • Participate in facility surveys and audits conducted by regulatory agencies • Maintain confidentiality of resident and facility information and report unauthorized access Committee Functions • Attend and participate in interdisciplinary committee meetings as assigned (e.g., Infection Control, QAA, Pharmacy) • Assist in preparing reports and presenting information to committees • Participate in care plan and discharge planning meetings Personnel Functions • Ensure adequate staffing of licensed nurses and CNAs for assigned shift • Delegate duties appropriately and supervise nursing staff • Conduct rounds to ensure care is delivered according to standards • Monitor attendance and report staffing concerns • Participate in performance evaluations and disciplinary processes • Foster teamwork, professionalism, and a positive work environment • Report suspected fraud, abuse, or safety concerns promptly Nursing Care Functions • Provide direct nursing care as required • Perform resident assessments and monitor changes in condition • Administer medications and treatments per physician orders • Ensure accurate documentation of nursing care and resident responses • Coordinate care with physicians, families, and interdisciplinary team members • Respond to emergencies and changes in resident status • Ensure medication administration and treatment schedules are followed • Monitor infection control practices and resident safety Staff Development • Participate in and support in-service education and training programs • Assist with orientation and supervision of nurse aide trainees • Maintain required continuing education and licensure Safety and Infection Control • Enforce safety, sanitation, and infection control policies • Ensure proper use of PPE and adherence to exposure protocols • Participate in emergency preparedness, fire drills, and disaster response • Report hazardous conditions and safety violations Care Planning & Assessment • Participate in resident assessments, care plans, and MDS processes • Ensure nursing staff follows resident care plans • Review documentation to ensure compliance and quality care Supervisory Responsibilities This position assists with the overall supervision and management of nursing staff and is delegated administrative authority, responsibility, and accountability for assigned duties. Qualifications Education & Experience • Graduate of an accredited nursing program • Minimum of 2 years RN experience, preferably in a skilled nursing or long-term care setting • Training in rehabilitative and restorative nursing practices preferred Licensure & Certifications • Current, active, and unencumbered California RN license • Current CPR certification • Knowledge and experience with PointClickCare (PCC) preferred Skills & Abilities • Strong leadership, clinical judgment, and decision-making skills • Excellent communication and interpersonal skills • Ability to work effectively with residents, families, staff, and regulatory agencies • Ability to remain calm under pressure and manage multiple priorities Physical Demands • Ability to move throughout the facility during the workday • Ability to lift, push, pull, and carry up to 25 lbs routinely and more with assistance • Ability to cope with emotional and physical demands of resident care • Ability to assist in emergency evacuations if necessary Work Environment • Skilled Nursing Facility setting • Exposure to infectious diseases, chemicals, and medical equipment • Frequent interaction with residents, families, staff, and healthcare professionals Equal Employment Opportunity (EEO) Statement Hanford Post Acute is an Equal Opportunity Employer.
