Schedule: Full Time - 80 hours/pay period – Monday through Friday – 8 a.m. to 4:30 p.m.
Shared Communities: This position is currently based at The Plaza in Anoka and will expand in Spring 2026 to support both The Plaza in Anoka and The Levande in Cambridge. The schedule remains Monday through Friday, with three days in Anoka and two days in Cambridge each week.

As an RN, you’ll play a key role in ensuring residents receive individualized, compassionate care that supports their highest level of independence and well-being. You will lead and support the delivery of high-quality clinical care to residents across our assisted living and memory care communities. This role is responsible for assessing and addressing resident needs, overseeing care coordination, mentoring staff, and ensuring regulatory compliance.
As an RN, you will:
• Clinical Oversight & Care Planning – Complete assessments, develop person-centered service plans, and monitor residents’ health status in accordance with state and federal regulations. Ensure timely documentation in the EHR and follow-up on significant changes in condition.
• Staff Leadership & Supervision – support with training, evaluating, and mentoring licensed and unlicensed staff. Oversee onboarding, performance reviews, delegation of tasks, and ensure team member compliance with regulatory and HIPAA standards.
• Resident & Family Collaboration – Serve as a clinical liaison for residents and families. Participate in care conferences, advocate for resident needs, and foster trust and open communication throughout the care journey.
• Compliance & Quality Improvement – Monitor infection control, incident reporting, and medication management. Partner with Director of Clinical Care and quality teams to support audits, staff training, and community compliance.
• Other Duties as Assigned – The duties and responsibilities listed above are representative of the nature and level of work assigned and are not necessarily all inclusive.
This job also requires:
• Current RN license in the state of Minnesota.
• BLS certification (or willingness to obtain within 6 months).
• Minimum of two years of experience in Assisted Living, Memory Care, or Long-Term Care preferred.
• Strong leadership, critical thinking, and organizational skills.
• Familiarity with MN Assisted Living regulations and Electronic Health Records (EHR).
• Ability to pass state mandated background checks.
• Physical capability to perform all essential job functions.
• Ability to read, write, and speak English to ensure effective communication with team members, residents, and families.
