Responsibilities
• Conduct comprehensive assessments and develop individualized care plans
• Deliver skilled nursing care in accordance with physician orders and organizational standards
• Monitor patient status, evaluate responses to treatment, and document all care accurately
• Educate patients and families on disease management, medications, and home safety
• Collaborate with interdisciplinary team members to ensure continuity of care
Qualifications
Education:
• Must have an Associates Degree in Nursing. (Must obtain BSN within 12 years of hire date)
Certification/License:
• Active Iowa Registered Nurse licensure issued by the Iowa Board of Nursing
• Within 1 year of hire, need to obtain RN licensure in the State of Wisconsin
• Must have a valid driver's license and proof of automobile insurance coverage
