#RN RN Home Health 40hrs weekly pay $2700 Multiple Locations in New Hampshire Job Description: • Provide compassionate end-of-life nursing care to patients in their homes or facilities. • Conduct initial and ongoing patient assessments to determine care needs. • Develop and update individualized care plans in collaboration with the interdisciplinary team. • Administer medications, manage symptoms, and ensure patient comfort. • Educate and support patients and families on disease progression, comfort measures, and care routines. • Coordinate with physicians, social workers, and other team members to ensure continuity of care. • Document all patient care activities accurately and timely according to hospice standards. • Participate in on-call rotation and respond to urgent patient needs.
