The ideal candidate will be a highly motivated self-starter with excellent clinical skills and a passion for helping patients maintain their independence and well-being. This position is open to both local and traveling RNs who are flexible to serve patients in the Albuquerque and Santa Fe areas. Responsibilities • Provide skilled nursing care to patients in their homes. • Conduct patient assessments and develop care plans. • Administer medications and treatments. • Educate patients and families on health management. • Complete accurate electronic health record (EHR) documentation, including OASIS. Qualifications: • Current and unrestricted RN license in New Mexico (or a Compact State). • Minimum 1 year of clinical experience as an RN. • Prior home health or hospice experience is preferred. • Experience with EHR and OASIS documentation. • Current BLS/CPR certification. • Valid driver's license and reliable transportation. 13 weeks FT available, Mon - Fri, 8am - 5pm, 40 hours per week 28 units Must cover both areas in Santa Fe and North ABQ as needed. ABQ area, may travel to Santa Fe for 1 week on call per month and or holidays 30 units HH and HCHB required It will be for north ABQ and SF proper to start with, once my new PRN is trained for ABQ it will be Santa Fe only. On Call is Mon-Sun and covers both areas, but for SOCs on the weekend it will be either area. We won't send to both areas same day. No guarantee, FT available Qualifications: HCHB, Oasis, and HH experience required Skills Homecare Homebase Software Experience - Required OASIS Experience - Required Current state license as a Registered Nurse - Required CHPN Certification - Preferred Proof of current CPR certification - Required Current driver's license - Required Must have reliable transportation and insurance - Required 1-2 years experience as a Registered Nurse in a clinical care setting - Required 2+ years Home health experience - Required.
