
RN Home Health - Colchester, VT in Burlington, VT
Job Description
Provide compassionate, skilled care to a diverse patient population in a home health setting. This contract opportunity is ideal for a licensed Registered Nurse with at least one year of clinical experience in home health nursing. You'll manage a caseload of patients ranging in age from 30 to 100 years, with diagnoses including cardiac, pulmonary, diabetes, wound care, neurological impairments, and palliative care among others. This role offers a structured 8 am to 5 pm shift schedule with minimal weekend and holiday requirements, allowing for a balanced work-life experience.
Qualifications and Desired Experience:
• Current RN licensure with a minimum of 1 year clinical experience in home health nursing
• BLS certification required; OASIS experience preferred
• Proficiency with patient care protocols including phlebotomy, IV therapy, wound vacs, feeding tubes, and catheter care
• Experience documenting care in electronic health record systems; training provided for Netsmart platform
• Ability to work within an average caseload of 18-22 clients and operate within a defined geographic area
• Professional and business casual attire with adherence to dress code policies
Responsibilities:
• Conduct home visits to provide nursing care and assessments
• Coordinate with CNA/PCT and unit clerical staff as part of the care team
• Manage clinical documentation and maintain compliance with established protocols
• Communicate effectively with patients, families, and interdisciplinary teams
• Utilize clinical equipment provided, including laptop and iPhone, to support patient care
