Job Description
This opportunity calls for a Registered Nurse (RN) with a strong background in home health nursing to provide and coordinate skilled nursing care directly in patients' homes. The role involves implementing physician orders through the nursing process, supervising and evaluating Home Health Aides and Licensed Practical or Vocational Nurses, and maintaining compliance with both agency policies and state-specific regulations.
Key Responsibilities:
• Assess and evaluate patient medical conditions regularly, adapting care plans as needed.
• Record all communications and clinical notes accurately in patient medical records.
• Supervise Home Health Aides every 14 days and licensed nurses per state requirements.
• Follow state-specific nursing regulations, including periodic patient reassessment and care plan revisions.
• Maintain current knowledge of professional nursing standards and participate in ongoing agency education.
Qualifications and Desired Experience:
• Current, active, and unencumbered RN license valid in the state of assignment.
• Nurse Licensure Compact (NLC) documentation if practicing in NLC states.
• Minimum 2-3 years of nursing experience, preferably with home health expertise.
• Current CPR certification.
• Proficiency with computers, including electronic health records and online systems.
• Ability to assess patients age-specific needs effectively.
• Graduates from accredited nursing programs; BSN preferred in certain states.
