We are a well known SUCCESSFUL Home Health Organization located in the Chicago Land area We are in need of a Home Health RN (Registered Nurse) to conduct patient in home visits in the SOUTH SUBURBS Competitive pay and flexible hours! Salary range: $88,000-$89,000 per year plus full benefits and a company car after 90 days is available for you if you want to use instead of using your own vehicle FULL TIME: approx 25 patient home visits per week LOW ACUITY patients, in thisposition you will do lots of educating on COPD, and Medication Management Area of coverage: SOUTH SUBURBS MAKE YOUR OWN SCHEDULE ENJOY LOTS OF FREEDOM IN THIS POSITION! Join our growing team of healthcare professionals as a home health nurse Home Health Nurse RN Responsibilities: • Completing nursing process in delivery of patient care which includes: assessment, planning, implementation, and evaluation of patient outcomes. • Total oversight of patient from admission to discharge. • Developing plan of care based on findings. • Serving as patient’s advocate and maintaining ongoing communication with physician. • Timely completion of all skilled nursing documentation Qualifications: • Current Illinois State RN License • Valid Drivers License and Reliable Vehicle • MINIMUM of 6 months Experience as a HOME HEALTH VISITING RN • Must be RELIABLE and DEPENDABLE • VALID CPR CERTIFICATION Apply here and we will be in touch with you soon!
