Job Description
Provide expert home health nursing care to patients and families in Middlebury, VT, employing your skills to improve patient outcomes in a supportive and collaborative environment. This contract position requires OASIS experience and engagement with a full spectrum of home health duties designed to meet regulatory standards and enhance quality care delivery.
Responsibilities:
• Conduct comprehensive patient and caregiver assessments utilizing the nursing process to develop and implement individualized plans of care that comply with state and federal regulations.
• Serve as Case Manager, coordinating care and managing caseloads to meet agency productivity standards.
• Complete OASIS documentation accurately and timely, maintaining up-to-date knowledge of documentation requirements.
• Implement nursing interventions including advanced wound care, IV therapy, and infection control practices.
• Collaborate with Licensed Practical Nurses and interdisciplinary team members to deliver cohesive and effective care.
• Provide patient and caregiver education on medication, treatment options, discharge instructions, and related health topics.
• Coordinate patient care transitions, ensuring thorough communication and proper documentation for safe discharges or transfers.
• Participate in peer review audits and interdisciplinary team meetings to continually enhance quality of care.
Qualifications:
• Registered Nurse licensure with current Vermont State Board of Nursing credentials.
• Proven experience in home health nursing with proficiency in OASIS documentation.
• Skilled in advanced nursing techniques such as IV use, wound VAC, and complex wound care.
• Strong organizational and communication skills necessary for effective case management and interdisciplinary collaboration.
• Ability to work independently while adhering to agency policies and regulatory standards.
