An experienced Registered Nurse with a focus on Home Health is sought for a contract position in Northfield, VT, working with a well-established home health agency. Key Qualifications:
• Current RN license with home health nursing experience
• Proficient in OASIS documentation and assessment
• Skilled in case management and patient care coordination
• Experience as a travel nurse is strongly preferred
• Reliable availability with no requested time off for the duration of the contract
Responsibilities:
• Conduct comprehensive patient assessments in the home setting
• Implement and manage individualized care plans
• Complete accurate and timely OASIS documentation
• Collaborate with interdisciplinary teams to ensure high-quality patient outcomes
• Provide patient and family education on health management
