This contract position offers a full-time schedule over 13 weeks, providing skilled nursing care to patients in their homes. Key Responsibilities:
• Deliver skilled nursing care and interventions in patient homes
• Conduct thorough patient assessments and complete OASIS documentation accurately and punctually

• Manage wound care, medication administration, patient education, and monitor chronic conditions
• Communicate significant updates and changes in patient conditions to the clinical team
Qualifications & Desired Experience:
• Active Registered Nurse licensure
• Minimum of 1 year recent experience in home health nursing
• Proven experience with OASIS documentation is required
• Proficiency with HomeCare HomeBase is highly preferred
• Ability to manage a structured, full-time schedule with daily patient visits
• Reliable transportation and valid drivers license
Schedule:
• Full-time, Monday through Friday
• On-call availability likely required
