Key Responsibilities: • Conduct skilled nursing assessments using OASIS documentation standards. • Provide compassionate home health and hospice care. • Coordinate and manage travel between patient visits efficiently. • Adapt to scheduling changes with professionalism. • Maintain accurate electronic medical records using HealthWyse (Careficient). • Participate in emergency call coverage on a limited basis. • Adhere to facility protocols including those related to background checks. Qualifications and Experience: • Registered Nurse (RN) licensed to practice in Vermont. • Proficiency with OASIS assessment and documentation. • Previous home health and hospice experience required. • Valid driver's license with a strong driving record. • Basic Life Support (BLS) certification mandatory. • Skilled in outpatient care delivery within community settings. • Comfortable using EMR systems, preferably HealthWyse or Careficient. Schedule: • Monday through Friday, 8:00 am to 5:00 pm. • Some weekend coverage and emergency call duties as needed.
