An opportunity is available for an experienced Registered Nurse (RN) specializing in home health to provide compassionate, patient-centered care on a contract basis. This role requires a strong background in home health nursing with specific experience using OASIS assessments. Key Responsibilities: • Deliver comprehensive nursing care to patients within their homes, focusing on individualized treatment. • Conduct initial and ongoing patient assessments, including vital signs, medication management, and wound care. • Create and implement tailored care plans based on patient needs. • Educate patients and family members on disease management, treatment options, and preventive health practices. • Collaborate with physicians, therapists, and healthcare teams to coordinate care plans. • Accurately document all patient interactions and care activities promptly in electronic medical records (EMR). • Monitor patient conditions and communicate any significant changes to the supervising physician. • Adhere to all state and federal regulations governing home health care services. Qualifications: • Active Registered Nurse (RN) license. • Minimum of three months home health experience utilizing OASIS. • Experience with Homecare Homebase (HCHB) software is highly advantageous. • Proficient clinical skills in IV therapy, wound care, and medication management. • Skilled in electronic medical records (EMR) systems. • Strong communication and interpersonal abilities. • Ability to work independently and efficiently manage time. • Valid drivers license and reliable transportation required.
