RN Hospice Admissions 7 on 7 off (after hours) in Phoenix, AZ

Full-Time Hospice RN – Admissions (7 on / 7 off) Stillwater Hospice Phoenix, Arizona (Field-based / On-call rotation included) Stillwater Hospice is one of the fastest-growing hospice providers in the country, delivering compassionate, high-quality end-of-life care to patients and their families across Arizona. We are committed to clinical excellence and are seeking a skilled, compassionate Hospice RN – Admissions to be the first point of clinical connection for patients entering hospice care. This role is ideal for an experienced nurse who thrives in an autonomous, mission-driven environment and values meaningful patient interactions while maintaining strong clinical standards. Key Responsibilities: • Conduct comprehensive hospice admission assessments in patients’ homes, facilities, or hospitals in accordance with hospice regulations and standards of care. • Evaluate patient eligibility for hospice services and collaborate with physicians, intake, and interdisciplinary team members to ensure appropriate admissions. • Complete accurate, timely clinical documentation, including assessments, care plans, and admission consents. • Educate patients and families on hospice philosophy, services, benefits, and expectations with empathy and clarity. • Coordinate care transitions and communicate effectively with referral sources, case managers, and internal team members. • Ensure compliance with Medicare hospice regulations, agency policies, and documentation requirements. • Participate in the interdisciplinary team process and support continuity of care during admissions. • Rotate on-call responsibilities as assigned during the 7 on / 7 off schedule. Qualifications: • Active RN license in the state of Arizona (required). • Minimum of 1–2 years of hospice, home health, or relevant clinical nursing experience preferred. • Strong assessment skills, critical thinking, and clinical judgment. • Ability to work independently, manage time effectively, and adapt in a fast-paced, field-based environment. • Excellent communication skills with a compassionate, patient-centered approach. • Familiarity with hospice EMR systems (e.g., WellSky, Homecare Homebase, or similar) preferred. • Valid driver’s license, reliable transportation, and ability to travel within the service area.
