Home Care/Hospice Registered Nurse
Assignment Duration
This is an 8-week contract position from January 5th to February 27th.
Schedule
Monday to Friday, 8:00 AM to 5:00 PM.
Job Description
We are seeking a dedicated Home Care/Hospice Registered Nurse to provide compassionate care to patients in a home setting. The position involves case management and the management of a patient caseload, necessitating travel throughout the day. Candidates must be equipped with HCHB experience and a willingness to utilize their personal vehicle for work-related travel.
Responsibilities
• Manage and coordinate care for hospice patients in their homes.
• Work collaboratively with a multidisciplinary team to develop and implement care plans.
• Travel to patient homes daily, providing nursing care and emotional support.
• Document patient progress and maintain accurate records as required.
Required Experience
• Minimum of 1 year case management experience is required.
• Experience with HCHB is required.
• Must have a valid nursing license and reliable transportation.
