Schedule
Shift: 7 AM to 7:30 PM for inpatient care and 8 AM to 4:30 PM for home hospice. Flexibility to work weekends required.
Job Description
The Hospice Registered Nurse will provide compassionate end-of-life care to terminally ill patients and their families. Responsibilities include complex symptom management, medication administration, and comprehensive patient assessments. The role requires strong communication skills to facilitate care discussions with patients and families, while ensuring comfort and support during their care journey.
Responsibilities
• Manage a caseload of patients in a home setting, providing individualized care.
• Conduct comprehensive assessments and develop care plans in collaboration with the healthcare team.
• Administer medications via various routes including IV, subcutaneous, and oral.
• Facilitate goals of care discussions and planning with patients and families.
• Provide education and support to patients and caregivers regarding end-of-life care.
• Utilize specialized medical equipment and monitor essential client health metrics.
Required Experience / Certifications / Licensure
• Minimum 3 years of community hospice experience.
• BLS certification required.
• Strong typing skills and familiarity with electronic medical records (EMR).
• Ability to manage complex symptom assessments and provide critical thinking in patient care scenarios.
