RN: Hospice in Newport, VT

Home Health and Hospice Nurse Schedule Monday – Friday, 8 am – 5 pm with some weekend coverage required. Some call may be scheduled, but only on an emergency basis. Job Description We are seeking a dedicated Home Health and Hospice Nurse to provide exceptional patient care in their homes. The ideal candidate will be experienced in OASIS assessments and will have a strong background in home health and hospice services. Responsibilities • Conduct home visits and provide comprehensive nursing assessments. • Manage patient care plans and coordinate services with other healthcare professionals. • Document patient progress and update electronic health records using HealthWyse. • Adapt to changing schedules and prioritize patient needs. Required Experience / Certifications / Licensure • Must have OASIS (Outcome and Assessment Information Set) experience. • Home health and hospice experience required. • Experience traveling from patient to patient for home visits. • Must have a driver’s license in good standing. • BLS certification required.
