Job Description
The Home Health/Hospice RN provides medical care and support to patients in their homes, focusing on their individual needs and recovery goals. This role emphasizes compassionate and comprehensive care for terminally ill patients and their families, prioritizing comfort, quality of life, and end-of-life support.
Responsibilities
• Assessing patient needs and developing care plans
• Administering medications and providing education to patients and families
• Collaborating with other healthcare professionals to ensure comprehensive patient care
• Monitoring patient conditions, ensuring comfort and safety
• Documenting care provided accurately and timely
Required Experience / Certifications / Licensure
• Experience in both Home Health and Hospice settings
• Current Montana RN license
• Clean driving record and valid driver’s license
• BLS certification
