Responsibilities
• Provide direct patient care in an intermediate care setting, monitoring vital signs and patient status
• Administer medications and treatments according to physician orders
• Collaborate with healthcare team to develop and implement patient care plans
• Educate patients and families on health conditions and post-discharge care
• Document patient information accurately and timely
