Join our team as Registered Nurse, MDS Coordinator in our Long Term Care in Wabasha, MN and be the change you want to see in the world. What's Available • 0.8 FTE (32 hours/week) • This will be a Day/Evening rotation and include regular hours as MDS Coordinator, as well as RN, LTC role. • Schedule will include weekend commitment of every 4th weekend combination of on-call or hours worked. What You Will Do • Provide LOVE + MEDICINE to our patients and residents • Responsibilities will include ensuring and assisting with compliance with regulatory requirements and maintaining the quality of resident assessments in accordance with the Minimum Data Set (MDS) guidelines. • Playing a critical part in enhancing the quality of care for residents by ensuring accurate documentation and effective communication across interdisciplinary teams. • The registered nurse (RN) focuses on the health and wellness of individuals, families and populations in collaboration with other health care professionals by incorporating evidence-based practice in the management of acute illness, chronic disease, disability, and end-of-life care. • The RN utilizes the nursing process and aligns practice with organizational policies, professional nursing standards and regulatory practice acts • Partner with a team of providers, medical professionals, and managers who are here to support your success and that of our patients in their health journeys. What You Need • Minimum of an Associate degree in Nursing • RN licensure to practice in the state of Minnesota upon hire • Current Infant-Adult Basic Life Support for Health Care Providers • Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare team members • Ability to work independently as well as collaboratively within a team environment We inspire your best life by relentlessly caring, learning and innovating.
