Deliver advanced nursing care in a dynamic home-based setting, supporting patients through a hybrid role combining command center responsibilities and in-home visits. Qualifications & Desired Experience: • Current RN license with Medical-Surgical, ER, or ICU experience • Ability to manage care both remotely and in patient homes • Strong clinical assessment and patient management skills • Comfortable working a rotating schedule including weekends • Ability to travel intermittently between patient homes within the assigned region Location: Bear, Delaware Bear offers a suburban atmosphere with easy access to larger metropolitan areas.
