POSITION SUMMARY:

Preserves, supports, and restores health and wellness by practicing at the highest level of the Registered Nurse licensure; collaborates with providers and multidisciplinary care team members to provide care coordination across the continuum. Assesses and supports physical symptoms, provides psychological support to patients, friends, and families, mentors assigned team members, promotes best practices, and always maintains a professional disposition.

DUTIES & RESPONSIBILITIES:
• Assesses the patient systematically utilizing verbal and technical nursing skills to establish a basis for patient care.
• Utilizes the EMR for all aspects of patient care to plan and document care, education, interventions and responses pertaining to patient care including medication reconciliation and administration, problem lists, care plans, assessments, histories, discharge planning, teaching and education which includes patient and family participation. Performs review end of shift.
• Coordinates referrals through Charge Nurse with physicians, outside agencies and other departments. Recognizes needs of inpatients vs. swing bed patients. Oversees and coordinates daily care of assigned patients during shift.
• Follows nursing protocol in assuring proper format is in compliance with standards of care and within scope of practice.
• Initiates an effective working relationship by utilizing interpersonal communication techniques with health care delivery team and coordinates patient care with staff assistants (LPN’s, CNA’s and Ward Clerks).
• Maintains an appropriate communication system through documentation and interaction with patient, significant others and members of the health care delivery team. Acts as a patient advocate.
• Coordinates learning activities and functions as a role model and resource person for students and staff including orientation of new personnel.
• Demonstrates leadership skills in coordinating staff and patient’s activities by effective planning, time management, delegation and feedback.
• Demonstrates initiative in identifying opportunities for self-development, personal and professional goals and objectives in response to self-evaluation
• Contributes to a positive work climate and to the overall team effort of the nursing unit.
• Adheres to the Behavioral Standards policy.
• Assists in orientation of new staff.
• Supervises and teaches procedures to less skilled personnel.
• Performs related responsibilities as required and directed. Willing and able to work in:
• Medsurg, ED, Triage, and Charge Nurse as trained.
• Takes initiative to keep skills, licenses and certifications up-to-date in all areas assigned.
• Is familiar with and coordinates the transfer process and EMTALA regulations as applicable.
• Participates actively in committees for improvement of nursing care. Actively works in Quality Improvement areas.
• Participates in creating departmental goals as requested by Manager.
• Reviews Communication Board to keep current on departmental and organizational performance and information.
• Attends all mandatory in-services and meetings as required to do so. Participates in Mandatory hospital wide staff education requirements, example: Care Learning, Infection Control requirements, Competencies, etc.
• Takes initiative to keep informed in all changes occurring in department by attending staff meetings. Gives input into meetings. Reads minutes of meetings if unable to attend. Brings concerns, questions forward to discuss for resolution. Willing to be active in the resolution process.
• Makes patient rounds to gather information and keeps updated on patient conditions by seeking information on a continuing basis. Relays all pertinent information to the Charge RN. Give pertinent and informative report to next shift utilizing current shift report policy. Performs bedside shift report according to current policy.
• Participates in hourly rounding and adheres to policy
• Participates in bedside shift report and includes information according to policy.
• Keeps the Nursing Manager and Chief Nursing Officer informed on the progress of all new personnel during orientation/probation period.
• Gives input into staff evaluations as asked by the Nursing Manager.
• Keeps the Charge RN or Nursing Manager informed of any ongoing staff problems or complaints that need intervention.
• Notifies Charge Nurse of need to interact with patient or patient’s family at time when complaints or concerns are voiced.
• Keeps up-to-date with current EMR.
• Knows procedure for calling/paging of on-call personnel and physicians.
• Knows role in the event of a fire, disaster or code. Applies the same in a drill situation.
• Recognizes and follows the Chain of Command.
• Performs other duties as assigned.

Graduate of an approved school of professional nursing.

RN with a current license to practice as a professional nurse in the State of Michigan.

BLS, ACLS and PALS, TNCC certified within one year of hire. NRP and ENPC recommended

Experience working in Medical-Surgical and ED areas preferred.

Knowledge and experience in documenting in an Electronic Medical Record (EMR).

Complete understanding and compliance with all HIPAA regulations.
