This opportunity invites experienced Registered Nurses to provide comprehensive home health care in a contract role based in Galax, VA. The position involves delivering skilled nursing services according to physicians orders, ensuring compliance with agency policies, and applying nursing processes with clinical expertise. The role includes direct supervision and evaluation of Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses, maintaining the highest standards of patient care.

Responsibilities:
• Conduct patient assessments and evaluate medical conditions, reporting significant changes promptly.
• Develop, implement, and regularly update individualized plans of care aligned with patient needs.
• Supervise Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses according to state-specific regulations.
• Maintain accurate documentation of patient status, interventions, and communications.
• Provide patient and family counseling related to care plans and health needs.
• Participate in agency in-service education and promote best practice standards.
• Coordinate and oversee all nursing services delivered to patients.

Qualifications & Desired Experience:
• Current, unencumbered nursing license in the assigned state.
• Eligibility for Nurse Licensure Compact (NLC) for multi-state practice if applicable.
• Minimum 2-3 years of professional nursing experience.
• Current CPR certification.
• Proficiency in patient assessment across age-specific needs.
• Basic computer skills including email, internet navigation, and use of online training and HR platforms.
• Valid drivers license, reliable transportation, and required liability insurance for home visits.
