Key Responsibilities:
• Conduct comprehensive, age-specific admission histories and physical assessments, continuously updating nursing care plans.
• Analyze clinical data including labs and diagnostics to tailor individualized care strategies.
• Identify and document residents at risk for nutritional and functional decline.
• Recognize and report signs of neglect or abuse.
• Prioritize, review, and adjust daily care plans in coordination with residents, families, and interdisciplinary teams.

• Communicate effectively with physicians to synchronize medical and nursing plans.
• Administer medications and IV therapy competently, managing urgent and emergent care situations promptly.

• Stay current with CMS regulations, Medicare skilled criteria, and documentation standards to ensure compliance.
