Job Description
This position offers nursing professionals the opportunity to provide and coordinate home nursing care in accordance with physicians orders and agency policies. The role involves direct patient interaction through home visits and requires the competent application of the nursing process to assess, evaluate, and manage patient care. Clinical oversight responsibilities include supervising and evaluating Home Health Aides and Licensed Practical/Vocational Nurses to ensure high-quality care delivery.
Key Responsibilities:
• Conduct comprehensive assessments of patients medical conditions and implement individualized care plans.
• Report significant changes in patient status promptly to physicians for medical intervention.
• Supervise and evaluate Home Health Aides and LPN/LVNs regularly, adjusting care assignments as patient needs evolve.
• Maintain accurate and timely documentation in patient medical records, including communication outcomes.
• Comply with all state- and federal-mandated regulations and professional nursing standards.
• Attend periodic office meetings and manage patient visits, requiring reliable transportation.
Qualifications and Experience:
• Current, unencumbered Registered Nurse license valid in the assigned state.
• Nurse Licensure Compact eligibility preferred for multi-state practice.
• Minimum 2-3 years of professional nursing experience, preferably with home health or related clinical settings.
• Current CPR certification.
• Proficient computer skills for electronic documentation and online training.
• Ability to assess patient status and tailor care to age-specific needs.
• Graduate of an accredited nursing program; BSN preferred in some states.
