Position Summary

The Trauma Performance Improvement - Nurse Navigator is responsible for providing clinical and system oversight for the care of the injured patient and assisting with quality improvement initiatives for trauma care services. The nurse navigator role involves analyzing trauma data, identifying areas of improvement and assist with implementing performance enhancing strategies. The Trauma PI Nurse Navigator works closely with Trauma Performance Improvement Coordinator and multidisciplinary care team to monitor outcomes, develop action plans, and ensure compliance with the national trauma standards, all while fostering a culture of continuous improvement of enhanced patient care. Additionally, The Trauma PI Nurse Navigator will serve as an advocate, utilizing the trauma-informed approach, for the patient, family members, trauma care services and hospital system. Having a well-developed understanding of trauma and multidisciplinary care processes, the navigator will guide patients and families, while mitigating concerns throughout the continuum of care.

Minimum Requirements

Education
• Bachelor's degree in nursing

Experience
• Two years of clinical nursing experience

License/Registration/Certifications
• Valid South Carolina Registered Nursing license
• BLS
• ACLS

Preferred Requirements

Preferred Education
• Master's degree in nursing

Preferred Experience
• Five years of clinical nursing experience

Preferred License/Registration/Certifications
• Trauma Certified Registered Nurse

Core Job Responsibilities
• Collaborates with the Trauma Program Manager (TPM), Trauma Medical Director (TMD), Trauma Performance Improvement Coordinator (TPIC), and multidisciplinary care team to provide clinical and system oversight for the care of the injured patient, ensuring provision of patient safety and proficient quality throughout the continuum of care.
• Serves as an advocate and educator for patients and families to understand the complexity of care, assist with team communication and navigating Trauma Care Services and the multidisciplinary care team.
• Performs daily rounds on trauma patients ensuring adherence to guidelines, algorithms, and protocols.
• Assess, review, and evaluate care identifying opportunities for improvement: PI audit filters, complications/ hospital events and tracking of pre-existing conditions.
• Collaborates with TPIC and assists with identification, investigation, reporting, and loop closure/follow-up of opportunities of improvement and quality issues throughout the program while maintaining confidentiality.
• Coordinates performance improvement projects with multidisciplinary trauma team, nursing, and all surgical subspecialty services.
• Conducts psychosocial and social vulnerability assessments to identify patient needs and coordinate appropriate interventions and resources.
• Provide clinical support, expertise, and training in trauma care for nurses, pre-hospital providers and other health care professionals.
• Participates in Trauma Multidisciplinary Team rounds to assess, review, and evaluate standards of care for the injured patient and quality of trauma processes.
• Assists in the development of nursing standards of care for trauma patients in collaboration with appropriate disciplines.
• Participates in the development, implementation, and ongoing assessment of trauma guidelines, algorithms, and protocols.
• Participates in multidisciplinary quality and performance improvement projects and/or teams.
• Utilizes TQIP reporting and benchmarking reports to implement quality and performance improvement initiatives to trauma patients.
