
RN Nurse Home Visitor – Nurse-Family Partnership (NFP) in Ball Pond, CT
Job Description
RVNAhealth is seeking a self-motivated, compassionate RN to join our Nurse-Family Partnership (NFP) team. This evidence-based, community health program transforms the lives of parents and their children by providing vital home-based nursing support from pregnancy through the child's second birthday.
As a Nurse Home Visitor, you will work directly with eligible individuals and families, delivering nursing education in the home setting. You'll play a pivotal role in promoting healthier pregnancies, stronger parent-child relationships, early childhood development, and long-term family success.
Position Highlights:
Visit clients in their homes
Work remotely between visits
Travel required within the Waterbury/Danbury region
Flexible schedule
Full-Time schedule of 37.5 hour per week
Occasional evening or weekend visits required to meet client needs (scheduled in advance by nurse; flex time provided)
Work with diverse populations impacted by economic and health disparities
Spanish-speaking candidates highly preferred
Education-based nursing position; minimal hands-on nursing care
Extensive training and support are provided
Essential Responsibilities:
Provide regular home visits to pregnant individuals and their families
Educate clients on prenatal health, nutrition, infant care, child development, and goal setting
Adhere to the NFP model and the nursing process
Carry a caseload of up to 25 clients
Participate in initial and ongoing NFP education
Collaborate with interdisciplinary partners and community resources
Qualifications:
Current RN license in good standing and CPR certification required at time of hire.
Two years of recent experience in maternal/child health, public health, behavioral health, or home visiting preferred.
Excellent written, verbal, and basic computer skills.
Spanish language fluency highly preferred.
Individual must be self-directed, enjoy flexibility, and be comfortable with variety and unpredictability in their daily work.
Must have a valid driver's license and vehicle to travel throughout the specified area.

Compensation:
Our target pay range for this position is $86,000 to $90,480 per year. This pay range represents RVNAhealth's commitment to pay transparency and is a reasonable estimate of possible compensation for this role at the time of posting. The final salary for this role will be determined based on careful consideration of a variety of factors, such as the candidate's exact location, experience, and expertise related to the qualifications.
