Job Title: Registered Nurse (RN) – Home Health Job Summary The Home Health Registered Nurse (RN) provides skilled nursing care to patients in their homes, promoting health, preventing illness, and supporting recovery. The RN works independently while collaborating with physicians, therapists, and other healthcare professionals to develop and implement individualized plans of care. Key Responsibilities • Conduct comprehensive patient assessments in the home setting • Develop, implement, and update individualized plans of care • Administer medications, injections, IV therapy, and wound care • Monitor patient condition and document changes accurately • Educate patients and families on disease management, medications, and self-care • Coordinate care with physicians, case managers, and interdisciplinary team members • Perform skilled nursing procedures according to physician orders and agency policy • Ensure compliance with state, federal, and accrediting body regulations • Maintain accurate, timely clinical documentation • Provide emotional support and advocacy for patients and families Qualifications • Current, unrestricted RN license in the state of practice • Graduate of an accredited nursing program (ADN or BSN) • Minimum of 1 year clinical nursing experience (home health preferred) • Valid driver’s license and reliable transportation • Current BLS/CPR certification Skills & Competencies • Strong assessment and critical-thinking skills • Excellent communication and patient education abilities • Ability to work independently and manage time effectively • Compassionate, patient-centered approach to care • Proficiency with electronic health records (EHR) • Knowledge of home health regulations and standards of care Physical Requirements • Ability to lift and move patients as needed • Frequent standing, walking, bending, and driving • Ability to work in varied home environments Work Environment • Patient homes within assigned geographic area • Flexible scheduling may include weekends or on-call rotation MOONLIGHT HOME HEALTH SERVICES is an EEO Employer - M/F/Disability/Protected Veteran Status View all jobs at this company
