Job Title Home Health RN Job Summary Registered Nurse needed for a Home Health assignment. The RN will be responsible for providing direct patient care in a home setting, managing patient caseloads, and collaborating with a multidisciplinary team to ensure optimal patient outcomes. Key Responsibilities • Providing skilled nursing care to patients in their homes. • Managing a patient caseload and coordinating care with other healthcare professionals. • Performing patient assessments and developing individualized care plans. • Administering medications and treatments as prescribed by a physician. • Educating patients and families on disease management and self-care techniques. • Documenting patient care accurately and timely in the electronic medical record (Netsmart). • Participating in on-call rotation as needed (2-4 times per month weekdays 5pm-8am). • Performing patient infusions, IV antibiotics, TPN, central line and PICC line care, and blood draws. • Working into holiday rotation. Required Qualifications • Active and unrestricted RN (Registered Nurse) license in California. • BLS Certification • Experience in Home Health. Preferred Qualifications • Experience with Netsmart (formerly Allscripts) EMR system. • Experience with pt infusions, IV antibiotics, TPN, central lines, PICC lines, blood draws. May have to do infusions for peds, if the visit is related to pediatric condition they will be sent to ER for evaluation, most/if not all of pt population is not peds.
