This role is vital to the medical success of those we serve, offering a unique blend of hands-on clinical care and medical oversight. You will act as a medical guide during crises and a consistent advocate for individuals during critical care appointments. In this role, you aren't just a nurse; you are a teacher and a coordinator who builds lasting relationships with medical providers and families to ensure well-managed, proactive healthcare is received. What YOU'LL Do: • Medical Monitoring: Monitor daily documentation for medical needs, identify health trends promptly, and perform health monitoring of documentation to ensure high scores in quality audits. • Care Coordination: Coordinate with pharmacies and doctors, complete medical appointments, and provide advocacy at critical care consultations. • Clinical Support: Provide hands-on care when needed, including wound care, ostomy care, catheter management, and monitoring critical care medical needs. • Education & Instruction: Teach medication administration refresher courses, lead clinicals for CMAs and Medication Managers, and provide on-site instruction for staff regarding care plans and medical protocols. • Administration: Update and maintain care plans and medical protocols while utilizing technology (Therap) for accurate documentation and regulatory compliance.
