As a Senior Hospice Care Manager, you'll be instrumental in overseeing branch-level clinical operations, managing direct patient care activities, and providing exceptional leadership to ensure compliance and quality in our hospice services. Key Responsibilities: • Report directly to the Executive Director or Administrator. • Lead and support a high-functioning hospice program, ensuring the delivery of exceptional care and adherence to regulations. • Supervise, instruct, evaluate, and nurture nursing and clinical staff to foster professional development and clinical excellence. • Strategically manage the scheduling and assignments of admissions and evaluation visits, ensuring timely care delivery by clinical staff. • Oversee clinical and administrative processes, such as documentation of referrals, insurance verifications, and patient benefit tracking. • Facilitate Interdisciplinary Group (IDG) meetings, including care plan updates and patient reviews. • Ensure proper documentation and communication between clinical and operational teams. • Monitor compliance with company policies, procedures, and regulatory standards. • Act as the primary liaison between clinical teams, leadership, physicians, and other healthcare providers for seamless care delivery. Qualifications - What You Bring: • Current RN license in the state of employment. • Graduate of an accredited nursing program. • Minimum of 3 years of direct patient care experience, with at least 2 years in hospice or home-based care. • Demonstrated ability to lead teams, manage clinical operations, and coordinate patient care. • Strong knowledge of hospice principles, industry regulations (Medicare, Medicaid, JCAHO, ACHC), and best practices. • Familiarity with managed care principles and human resource guidelines. • Leadership style that fosters empathy, quality, and operational excellence. Preferred Qualifications: • Bachelor of Science in Nursing (BSN). • Prior experience as a clinical supervisor or in program management. • CHPN certification (Certified Hospice and Palliative Nurse). • Proficiency in electronic medical records (EMR) systems and healthcare technology.
