Senior Nurse - Case Management in Las Vegas, NV

Your Role Join our dedicated Care Management team, where your expertise will contribute significantly to our mission of supporting patients in achieving optimal health and wellness in outpatient settings. As a Senior Nurse in Case Management, you will report to the Manager of Care Management and play a vital role in assessing patient needs, providing crucial clinical education, and coordinating care with providers, medical groups, and community resources. Your responsibilities will include managing and coordinating patient care to ensure that our members receive the highest quality of services. You will utilize your clinical judgment and independent analysis while collaborating with members and interdisciplinary teams, including clinical nurses and treating physicians. Your Knowledge and Experience • Current and valid RN license required; CA RN License preferred, with potential for compact state licensure. Must maintain active, unrestricted RN license in assigned state(s) or obtain required licensure within 90 days of hire. • Bachelor of Science in Nursing or advanced degree preferred. • Certification as a Certified Case Manager (CCM) is required or in the process of completing certification when eligible based on application requirements. • 5 years of experience in nursing, healthcare, or a related field is required. • A minimum of 3 years of managed care experience in either inpatient, outpatient, or managed care settings is preferred. • Experience in health insurance/managed care is also preferred. • Expertise in transitions of care is a plus. • Strong knowledge of healthcare delivery systems, managed care principles, and care coordination is essential. • Excellent communication skills are a must.
