The Role
The Skilled Nursing Registered Nurse plays a crucial role in delivering intermittent skilled nursing services while supervising and guiding hospice aide staff in providing personal patient care. As a Registered Nurse, your contributions will be vital in enhancing patient care.
Key Responsibilities:
• Evaluate and assess patient needs, establishing care plan goals collaboratively with patients.
• Communicate patient status, updates, and discharge planning to clinical leadership, attending physicians, and hospice physicians as necessary.
• Conduct annual on-site visits to patient care locations to observe and evaluate aide performance.
• Participate in staff meetings and educational in-service training.
• Ensure the integration of the 4Ms (What Matters, Medications, Mentation, Mobility) in delivering Age-Friendly Care.
Skills and Experience Required:
• Current unencumbered State Professional Nurse License.
• Valid driver's license with a good driving record.
• Ability to work effectively in a field setting and exercise sound nursing judgment.
• Expertise in assessing patient needs and crafting individualized care plans.
• Proficient with Microsoft Office applications.
