Role Description This is a Skilled Visit Nurse role located in Colorado Springs, CO. The Skilled Visit Nurse will deliver professional nursing care to patients in accordance with their individualized care plans. Responsibilities include monitoring and assessing patient conditions, providing education to patients and families, administering medications and treatments as prescribed, and coordinating care with healthcare team members to ensure optimal outcomes. Qualifications • Valid Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of Colorado • Proficiency in administering medications, conducting assessments, and creating care plans • Strong critical thinking and problem-solving skills • Excellent communication and interpersonal skills for interacting with patients, families, and healthcare teams • Basic computer literacy and familiarity with documentation software • Current CPR and First Aid certifications • Experience in home health or clinical nursing is preferred • Compassionate approach with the ability to provide patient-focused care
