Job Title: Registered Nurse (RN) Location: Fort Smith Department: Nursing Reports To: Director of Nursing Employment Type: Full-Time Position Summary: We are seeking a compassionate and dedicated Registered Nurse (RN) to provide essential care and support to patients in a home health setting. The RN will assess health needs, implement and manage care plans, educate patients and families, and work collaboratively with our interdisciplinary team to ensure quality care for all our patients. The ideal candidate is experienced, dependable, patient-focused, with strong communication skills and a heart for helping others maintain their independence at home. Key Responsibilities: • Patient Care: Assess patient health, follow individualized care plans, and provide direct nursing care. • Monitoring: Record symptoms, vital signs, and changes in condition. Update and maintain accurate medical records. • Collaboration: Coordinate with physicians, aides, therapists, and other healthcare team members to deliver cohesive care. • Supervision: Conduct supervisory home health aide visits and ensure care plan adherence. • Education: Provide patient and family education on medications, treatments, disease prevention, and health maintenance. Participate in in-service meetings and IDG meetings. • Medication Management: Administer medications and monitor for side effects. Deliver and manage medication as needed. Provide the means for caregivers to destroy unused controlled drugs according to policy. • Documentation: Complete care reports, IDG meeting updates, and compliance-related documentation within company's EMR system. • Safety & Supplies: Ensure a safe care environment; monitor and manage nursing supplies and equipment. • On-Call Rotation: Participate in scheduled on-call shift rotation. • Other Duties: Perform additional tasks as assigned to support the care team and patients. Requirements • Minimum of an Associate's degree in Nursing. Bachelor of Science in Nursing (BSN) preferred. • Current Registered Nursing license from the Arkansas State Board of Nursing. • Home health care experience preferred. • Valid driver's license. • Current CPR certificate. • Ability to travel out-of-the area and overnight as needed.
