You’ll work independently while being fully supported by an experienced clinical leadership team. This role is ideal for RNs who value autonomy, flexibility, and meaningful one-on-one patient relationships. Requirements Key Responsibilities • Complete Start of Care assessments and ongoing patient evaluations • Deliver skilled nursing services according to the plan of care • Educate and support patients and families • Coordinate care with physicians and interdisciplinary team members • Complete required documentation within agency guidelines • Supervise and support other nursing staff as needed • Participate in care conferences, peer review, and quality improvement initiatives (On-call rotation and OASIS responsibilities apply per agency policy.) Qualifications • Graduate of an accredited nursing program • Active RN license in the state(s) of practice • Home health or nursing experience preferred • Ability to work independently and exercise sound clinical judgment • Commitment to patient-centered care
