We are excited to announce an opening for a Telephonic Care Manager RN. We are seeking a dedicated Registered Nurse (RN) who is licensed and active, preferably with a compact license, to make a significant impact in the lives of our Medicaid/Medicare population in South Carolina. As a Care Manager RN, you will be responsible for conducting assessments, developing care plans, educating members, and connecting them to vital community resources. Your exceptional computer skills and attention to detail will be crucial as you multitask between various systems and ensure accurate documentation during member calls. This remote position requires a home office setup with high-speed internet connectivity, allowing you to work from the comfort of your home. We offer a supportive and dynamic environment, and we value productivity and member outcomes. Schedule: Monday through Friday, 8:00 AM to 5:00 PM EST, with flexibility to work one to two evenings until 6:00 PM EST. No weekends or holidays required. Key Responsibilities: • Conduct comprehensive member assessments to determine eligibility for care management. • Collaborate with members, caregivers, and physicians to develop and implement tailored care coordination plans. • Engage with members through telephonic or in-person visits as necessary. • Monitor and evaluate the effectiveness of care plans, documenting interventions and adjustments as needed. • Maintain a member caseload for regular outreach and effective management. • Promote the integration of services, especially in behavioral health and long-term supports. • Facilitate interdisciplinary care team meetings to ensure collaborative care delivery. • Utilize motivational interviewing techniques to inspire changes in member health behaviors. • Identify and address barriers to care to enhance member experiences and outcomes. • Provide guidance and resources to peers as appropriate. Qualifications: • Minimum of 2 years experience in healthcare, preferably in care management or related fields. • Valid and unrestricted RN license in South Carolina. • A valid driver’s license and reliable transportation are required for any necessary local travel (25-40%). • Strong understanding of electronic medical records (EMR) and HIPAA compliance. • Proficient in Microsoft Office and adept with various online portals. Preferred Qualifications: • Certification in Case Management (CCM) is advantageous. Molina Healthcare values its employees and offers a competitive benefits and compensation package.
