We are seeking a dedicated Hospice RN to join our compassionate team. In this unique and dynamic role, you will provide direct care to patients and vital support to their families while serving as a floating clinician assigned to various hospice branches based on staffing needs or census demands. As a Travel Hospice RN, You Will: • Deliver exceptional, compassionate nursing care for hospice patients. • Manage and implement personalized plans of care tailored to each individual. • Lead and collaborate with interdisciplinary team members (LPNs, CNAs, HHAs, etc.). • Assess and address patients' physical, emotional, spiritual, and environmental requirements. • Accurately document all care provided and ensure compliance with hospice protocols. • Educate families and caregivers on care instructions, medications, and safety practices. • Participate in the on-call rotation and provide after-hours care when necessary. • Collaborate effectively with physicians, referral sources, and the care team. • Assist with referrals, regulatory documentation, and payer compliance. • Travel to various Gentiva hospice locations as assigned to meet urgent needs. Schedule & Travel: This role generally follows a Monday-Friday schedule, with occasional on-call responsibilities. Travel is required, which may include: • Overnight travel required to cities within a 100-mile radius. • Frequent overnight travel to multiple states. • Regional travel with overnight stays based on assignment requirements. A company fleet car is provided for eligible employees. Qualifications - What You'll Bring: • Current, unencumbered RN license in the state of employment. • A minimum of 1 year of RN experience (hospice, home health, acute care, long-term care, oncology, or other high acuity settings). • Current CPR certification. • Valid driver's license, auto insurance, and reliable transportation. Preferred Experience (Not required): • Experience in hospice and end-of-life nursing. • Proficient in clinical assessments and compassionate communication. • Strong critical thinking, flexibility, and problem-solving skills. • Comfortable working in varied environments with changing needs. • Experienced in documentation and electronic health records. • Calm and supportive in crisis situations; skilled at engaging with families.
