License must be in hand at time of application. • Certifications BLS (AHA) • Specialty Experience 1+ years of experience in Rehabilitation in last 1 years. • Skills Assessment RHB skills assessment with 2.7+ score. • References 1 Supervisor reference within the last 1 year. 1 Charge RN reference within the last 1 year. • Additional Confirmations I have submitted all time off requests. I am aware that I cannot make additional requests after submission. • Flu Vaccine (Current Season) Must be for the current flu season. Medical & religious declinations are accepted.
