Job Summary
We are seeking an experienced Registered Nurse (RN) – Case Manager for an acute care setting. This role focuses on utilization review and care coordination, ensuring appropriate patient care, compliance with regulations, and effective discharge planning. The ideal candidate will have strong clinical judgment, excellent communication skills, and prior case management experience.
Roles & Responsibilities
• Perform utilization review to ensure medical necessity and appropriate level of care
• Coordinate patient care across multidisciplinary teams
• Facilitate discharge planning and transitions of care
• Communicate with providers, payers, and care teams regarding treatment plans
• Ensure compliance with hospital policies, payer guidelines, and regulatory requirements
• Maintain accurate and timely documentation in the electronic medical record
Reuirements:
• Active NY State of Registered Nurse (RN) license
• Minimum 2 years of RN experience
• Previous Case Management / Utilization Review experience preferred
• Strong knowledge of acute care workflows
• Proficiency with EPIC EMR
• Excellent communication, organization, and critical-thinking skills
