Requirements (Must-Have)
• Active RN license (current and in good standing)
• Minimum 2 years of experience
• OASIS (Outcome and Assessment Information Set) experience – REQUIRED
• Home Health AND Hospice experience – REQUIRED
• Experience traveling patient-to-patient for home visits
• Ability to manage and adapt to frequent schedule changes
• Valid driver’s license in good standing
• BLS/BCLS certification
