Schedule
• Shift: Day shift
• Hours: 5x8s (8:00 AM – 4:30 PM)
• Schedule Notes: Facility cannot accommodate 4x10s or 3x12s
• Weekend Requirement: May be required occasionally
Requirements
• Active RN license (OR or compact; pending license accepted)
• Minimum 1 year of skilled Home Health experience
• OASIS experience required
• BLS certification
• EPIC charting experience required/preferred
• Must have reliable transportation (travel within ~50-mile radius)
• Home Health first-timers not accepted
Additional Notes
• Ratios: 1:1
• Locals accepted (within 50 miles) at the same rate
• Guaranteed hours: Facility may call off up to 1 shift per pay period
• COVID vaccine: Facility guideline notes primary series + booster
• Scrubs: Any color
• Parking: Free
