Job Description: Travel Registered Nurse - Case Management
Are you an experienced and detail-oriented Registered Nurse looking to take your career on the road? We are seeking a skilled Travel Registered Nurse - Case Management professional to join our team. Key Responsibilities:
• Patient Coordination: Assess patient needs and develop individualized care plans to ensure timely and appropriate services are delivered throughout hospitalization and after discharge.
• Discharge Planning: Collaborate with physicians, therapists, and social workers to coordinate safe and effective discharge plans, including referrals to home care, rehabilitation, or long-term facilities.
• Utilization Review: Evaluate medical necessity and appropriateness of care using evidence-based guidelines to ensure compliance with payer requirements.
• Patient Advocacy: Act as a liaison between patients, families, and healthcare providers to promote patient-centered care and positive outcomes.
• Documentation: Maintain accurate, timely documentation of case management activities, including patient assessments, care plans, and progress notes.
• Resource Management: Identify community resources, support programs, and financial assistance options to optimize patient recovery and well-being.
• Travel Flexibility: Adapt to different healthcare settings and maintain excellence in patient management across diverse hospital systems.
Requirements:
• Active Registered Nurse (RN) license.
• Minimum of 2 years of experience in case management or discharge planning.
• Strong knowledge of healthcare systems, insurance processes, and resource coordination.
• BLS certification required; CCM (Certified Case Manager) preferred.
• Excellent organizational, problem-solving, and communication skills.
• Willingness to travel and work in various healthcare facilities.
