Requirements :
• Active RN license in the practicing state.
• BLS certification required.
• Prior home health, case management, or community nursing experience preferred.
• Strong assessment, clinical judgment, and care coordination skills.
• Ability to work independently and manage a home-visit caseload.
• Reliable transportation for patient visits.

Responsibilities :
• Perform in-home patient assessments and develop individualized care plans.
• Provide skilled nursing care including wound care, medication management, and health education.
• Monitor patient progress and adjust care plans as needed.
• Coordinate services with physicians, therapists, social workers, and caregivers.
• Document assessments, interventions, and patient outcomes accurately and timely.
• Educate patients and families on disease management, safety, and home care instructions.
• Ensure compliance with regulatory, agency, and quality standards.
