As an LTC RN on this assignment, you’ll be responsible for comprehensive care planning, precise medication administration, and proactive assessment of pain, delirium risk, skin integrity, and mobility. You’ll lead multi‑disciplinary rounds, update care plans, and coordinate with physicians, physical and occupational therapy, social work, and dietary services to achieve outcomes. You’ll implement evidence‑based protocols for fall prevention, infection control, and chronic disease management, while supporting rehab goals and successful transitions.
