Responsibilities • Provide high-quality nursing care to long-term care patients. • Participate in ongoing professional development and training. • Achieve performance targets and contribute to team success. • Collaborate with healthcare teams to develop patient care plans. • Monitor and document patient progress and changes in condition. • Educate patients and families on health management and care plans. • Adhere to safety and infection control protocols at all times. • Communicate effectively with patients, families, and team members. • Maintain accurate and timely documentation of patient care activities. • Utilize technology to track patient data and outcomes effectively. • Engage in quality improvement initiatives to enhance patient care. • Assist in the orientation and training of new staff members. Experience/Qualifications • Active RN license in the state of Maine with long-term care experience. • Commitment to ongoing professional development and continuous learning. • Strong communication skills with patients, families, and healthcare professionals. • Ability to meet performance targets and contribute to team success. • Ability to collaborate effectively with multidisciplinary healthcare teams. • Familiarity with safety and infection control protocols in healthcare. • Experience in monitoring and documenting patient care and progress. • Proficiency in utilizing technology for tracking patient data and outcomes. • Experience in quality improvement initiatives to enhance patient care. • Willingness to assist in training and orienting new staff members.
