
Traveling Hospice Registered Nurse in Brilliant, OH

About the Role: We are seeking a dedicated and compassionate Traveling Hospice Registered Nurse to join our team as a Regional Support specialist. In this dynamic position, you will provide direct care to patients and support to families while being adaptable to different hospice branches based on staffing needs. Key Responsibilities:
• Provide high-quality, empathetic nursing care to hospice patients.
• Manage and implement individualized care plans.
• Lead and support interdisciplinary team members including LPNs, CNAs, and HHAs.
• Assess and address patients' comprehensive needs - physical, emotional, spiritual, and environmental.
• Document all care and ensure compliance with hospice protocols.
• Educate families and caregivers on care instructions and medication safety.
• Participate in on-call rotations and provide after-hours care as needed.
• Collaborate with physicians and other care team members to ensure patient well-being.
• Assist with referrals, regulatory documentation, and payer compliance.
• Travel to various Gentiva hospice locations as needed to fill urgent care needs.
Travel Requirements: This role involves travel with options including:
• Overnight travel to locations within a 100-mile radius.
• Frequent overnight travel across states.
• Regional travel with overnight stays based on staffing needs.
A company fleet car will be provided for eligible employees.
Qualifications:
• Current, unencumbered RN license in the state of employment.
• Minimum of 1 year of RN experience in hospice, home health, acute care, or similar settings.
• Current CPR certification.
• Valid driver's license, auto insurance, and reliable transportation.
Preferred Experience:
• Previous hospice and end-of-life care experience.
• Strong clinical assessment and compassionate communication skills.
• Excellent critical thinking and problem-solving abilities.
• Comfortable adapting to shifting needs and diverse environments.
• Proficient in documentation and electronic health records.
• Ability to remain calm and supportive in crisis situations.
