Community Healthcare of Texas has provided Hospice and Palliative Care Services since 1996. Community Healthcare of Texas has cared for patients with serious and terminal illnesses throughout North Central Texas. POSITION SUMMARY
The Triage Nurse responds to all after-hours and weekend calls from all sites, prioritizes calls, and assigns crisis visits to on-call staff.
ESSENTIAL FUNCTIONS
• Responds to all calls, inquiries, pages from patients/families in a timely manner
• Discerns needs, advises, and prioritizes the need to assign on-call staff
• Remains calm under pressure with a customer service demeanor
• Follows up on requests
• Provides a complete report on all activities at each site following each triage day
POSITION QUALIFICATIONS
• Registered Nurse in the state of Texas
• Hold a valid driver’s license
• 1 year of nursing experience required
• Hospice/Home Health experience strongly preferred
• Must have proficient computer skills and Microsoft Office suite experience
