
Vermont Licensed Housing Case Manager in Anchorage, AK
Job Description
A company is looking for a Permanent Supportive Housing Case Manager.
Key Responsibilities
Determine eligibility for program services and conduct assessments to create person-centered care plans
Assist participants with accessing and renewing Medicaid
Coordinate services with local providers to ensure alignment with participants' needs and priorities
Required Qualifications
Master's degree in education, human services, health, counseling, or a related field, or a Bachelor's degree with one year of relevant experience
Strong commitment to housing first principles and trauma-informed care
Familiarity with housing and homeless systems of care and federal regulations (HUD / Medicaid)
Experience with community-based health / behavioral health and / or homeless assistance programs is preferred
Ability to pass background investigations as required for the position
